The Grade II listed war memorial in the village of Graffham in the South Downs, West Sussex is set back from the main road in a garden. Four steps lead up from the road and a wide paved path approaches the memorial which takes the form of a four sided granite stone column surmounted by an orb. The column is set on a square plinth with mouldings above and below, and has a two-stepped base. A low brick retaining wall runs alongside the road and beside the steps up to the path to the memorial.
In June 2005 War Memorials Trust made a grant of £250 towards hard landscaping work around the memorial. The retaining wall had deteriorated to the point that the safest option was for it to be rebuilt. The steps were reconstructed and the path to the memorial relaid.
On the front of the plinth is a dedication which reads

On the east side of the plinth are names of men who fell in World War I and below them, on the base moulding, are three men of the Wallace family dated 1939 -1945.
On the south side of the plinth are the names of six men who fell in World War I and below on the base moulding three from World War II.
On the west side of the plinth are nine names from World War I and below on the base moulding three names from World War II.
